Children's Support

To apply for this service, please download the PDF form (Ashley Denham Child fund application). 

The Ashley Denham Children's Lyme Support fund,   helps Oregon Children who are in need of care and insurance is not available (non insured or not covered).   Medical and financial need must be established  and funds go directly to the child' s LL physician depending on funds available.

or sending a check with "Ashley Denham's Children's Fund " in the memo line by regular mail to:
